In WOLF the nurses previously had an eForm with various questions and the "Fragility Index" calculator. They're still wanting this built.

Please see the attached for the information on how to calculate the FI score:
1. “Variable Coding Scheme for eFI-CGA” where you can find the list of the variables and the corresponding coding.
2. “FI Manual Calculation Sample” for some examples of calculations
a. In row 1, the number of variables are numbered and we have 55 variables total.
b. The formula we use to calculate FI is (sum of all variables)/ (54+18-missing values)
c. The denominator uses 54 instead of 55 because variables Mini-cog and MOCA count as 1 variable as we only include 1 of them in our calculation (please see row 6 and 7 for an example). So, if we have data for both mini-cog and MOCA, we would calculate FI using MOCA’s value. If we only have the mini-cog value and not MOCA, we would calculate FI using mini-cog value. So as long as we have 1 of mini-cog OR MOCA, the missing value count would not increase.
d. The 18 in the denominator is from # of problems as # of problems code = value of # of problems.
