Add ability to include height and weight data attached to a consultation request for all patients

– Impact : When dealing with certain at risk patients, height and weight data can be required to include with a consultation request, MYLE does not accommodate this inclusion.

– Current state: Currently in MYLE, the option to include height and weight data with a consultation request is only available in patient files where the patient is <19. This is valuable data, and this is an option that should be available to include with all patient files, not only for children.
