PA list: to be able to remove a pt form the list without using the ''to waiting room'' button

Users would need to be able to remove pts/appointments from the PA list without having to press on the ''to waiting room' button. This would be useful in cases where they want to not see phone appointments or time slots booked by doctors with no patients assigned.
