Able to send a form for the patient to consent to the Loi 25, before an appointment

Task affected: appointments and patients files

Impact: Client won't have to create a note to be able to import the form.

Current state: User needs to create a note and add the form, then send it to the patient so he or she can agree to the terms. Yet it could be sent before an appointment, just like the Covid questionnaire.
