Kiosk: More options in the "Personal Information Modification" section

Kiosk:

Users would like to have more options if they activate the "Personal Information Modification" option of their Kiosk environment.

Under the "Personal Information Modification" they to add 4 options:

1. An option called "Emergency phone number" they can select that will force the patient to enter a second phone number if case of an emergency (abnormal results, complications during an appointment, etc.) .

2. An option called "Close person email" they can select that will force the patient to enter the email address of a close person if case of an emergency (abnormal results, complications during an appointment, etc.) and if the patient doesn't have an email address in his file and refuses to provide one under his name.

3. An option called "Obligatory email" they can select that will force the patient to add an email address before they're able to confirm their presence.

4. An option called "Obligatory phone number" they can select that will force the patient to an email address before they're able to confirm their presence.
