Signature Doctor/Resident signature section on consultation and exam request enhancement / refinement

Client wishes to have the Consultation request and Exam request in notes made by Resident/Doctor form signature section reworked as currently there is no way to have both signature on the requests, only 2 possibilities can occur

1. Doctor signature with no mention of resident (even though resident made the request)

2. Resident can sign with the Doctor's line to sign staying empty

Also, whoever signs last will squash and automatically deletes the previous saved signed copy, regardless who signed first.
