Consent Programs: add a Refusal indication when the consent program is refused

As of now, when a patient refuses a consent program, no indication are listed in the patient's file. Only when the patient hasn't answered or consented will we see the indication. If we select '' patient has not answered yet'' and then change it for a refusal, it will appear but not when the consent program is added at first.

This makes it hard for the clinics to know if the patients have not consented to a program and forces them to proceed with more steps to obtain the desired result.
