Pediatric Patients enhancement/refinement regarding patient profile ie Address type, phone etc

Client wishes to be able to have Mother, Father as options in Type of address and for "Other Contact" to have a field underneath to state exactly who is the other contact.

Same for phone number, although note section currently is useful, they would like to have a field under "Type" when other is chosen aswell.

These requests are because they have many patients (mainly kids) that has 2 or more addresses (when in shared custody) and the current layout prevents them to accurately input all the necessary information with precision.
