Add an option to add the patient's medical summary when faxing an approved note

When using the "Fax" option on the top right side of an approved note, users would like to have an option to also include the patient's medical summary before faxing it.

This option will save some clicks since the users need to click on the printer icon, select the note and medical summary before faxing them.
