Modify the Medical Summary header when printing it

– Affected user(s): Any users creating medical notes.

– Task affected: Clinical notes, documents.

– Impact : It would allow clinics to show the specialist's name in the medical summary first instead of having the site's location featured as the header.

– Current state The specialist's name only appears at the bottom, in Confidentiality.
