Add refresh button in patient chart.

– Affected user(s): MOA/ doctors/ nurses

– Task affected: Refreshing patient chart

– Impact : Faster refresh / buy time / less clicks

– Current state users need to close patient chart and re-open ( Ex : Adding a patient to the list , it wont appear in the banner even if we click on another tab)
