Add patient's email to Exam Request letterhead

Update request for the Exam Request section in medical notes:

As long as the patient has an email address in his files (with "Notifications" selected or not), users want to see the email address of the patient included in his personal information when they print or fax the document.

Some professionals in BC won't call the patient, just send him an email.
