Ability to create forms bundle or combo

For several reasons, like pre-op for example, multiple forms have to be selected and filled at once.

Add the possibility to manage the forms, and create bundle could be a gain of efficiency for MD.

Ask the UROLOGIST for example in CCSMTL. At every consultation, they almost have to select 3 forms.
