Show who created the patient's enrollment in MYLE and the date it was done

– Affected user(s): Doctors, nurses, MOA, secretaries

– Task affected: Viewing a patient enrollment

– Impact : It would be able to trace back who did an enrollment when error slips in the patient's information.

– Current state Users can't tell who did the enrollment, unless going to the log tab and deducing it without a lot of information.
