Have a section 'Description' or 'Notes' when exporting private bill in CSV format

– Affected user(s): Users keeping tab on the billing

– Task affected: Creating a billing report

– Impact : It would allow for more accuracy on the billing. For example, user would like to be able to see the quantity of botox was used during the operation that lead to the bill.

– Current state User cannot have this information included when exporting the CSV file. They would need to add this information by themselves.
