DOCUMENTS: Notify the other user in a document that the document in question was approved and by whom.

When more than one clinician is assigned to a document, and one of them approves the document, the notification of a new document to be approved disappears for the other clinicians. For example, a laboratory report is assigned to the requesting physician and the CC physician.

Admins would like clinicians to be notified that the document was assigned to them, but approved by another clinician.
