RAMQ enrolement forms generated to depend on the type of enrolment

– Affected user(s): MOA, nurses or doctors doing the enrollment.

– Task affected: When the treating staff wants to generate the RAMQ forms in Enrolment sub tab in the patient banner.

– Impact: There is 3 forms generated:

• Inscription auprès d'un médecin de famille.

• Inscription auprès d'une infirmière praticienne spécialisée ou d'un infirmier praticien spécialisé en soins de première ligne (IPSPL).

• Formulaire de consentement au partage et à la communication des renseignements sur l'usager.

The client would like the forms genrated (to be printed) to show only the form for the doctor or the form for the IPS depending on the type of enrolment. She considers it will save paper and be more eco-friendly

– Current state: All the forms are printed but some forms are not used causing a waste of paper.
