Add the option to deactivate the 'Recently used in this file' for forms

– Impact : Avoid mistakes. Staff tend to click on Duplicate or Continue in the 'Recently used in this file' section , but if there has been an error in the demographic information, the error is added back to the form, which would not have been the case if they had chosen from the new forms.

– Current state They need to be careful when they are using a previous form used in the note before.
