Ability to modify / add information to the certificate when user is printing it

Patient file - History - Appointment - Print certificate.

Mrs Boucher from Clinique Médicale Mascouche would like to have the option to add information to the certificate that is automatically generated if some precisions need to be added and given to the patient.

The current workaround would be to either use an Exam request to be able to add information and give it to the patient, but in fact this is not an actual exam request, or to use a form to do it.

Client would like to have a small 'Notes' box where they can add important information and print it directly by clicking on print certificate
