– Task affected: In the patient's medical record note, when the doctor sees the patient again, this identification helps them gain a better understanding of the person they need to treat.

– Current state : For now, all the information related to the patient's identification and social habits are mixed together in a single section, making it difficult for doctors to discern at first glance.

– Competition consideration: Visually, it would be clearer, and it would take less time to find the information. Omnimed and Medesync include this type of section in their notes.
