Add standardization in the way forms are named in MYLE

Task affected: Adding forms in a clinical note

Impact: It would save a lot of time and allow users to find the correct form to use way quicker.

Current state: Right now, some form are called requête, demande or formulaires, have multiple repetition in their name, or are written in random orders
