This affects all professionals that evolve in an obstetrical environment.
The scope of the impact would depend on the level of attention users bring to their work flow.
Task affected:
On subsequent obstetrical visits of a patient, when wanting to update the original obstetrical file, users often end up creating a whole new obstetrical form instead of continuing the preexisting form using the Follow Up button.
Impact:
Due to the way obstetrical forms work, the fix for these mistakes can be time consuming.
There is also a risk of entering information in the wrong document, or losing the information when the erroneous forms are deleted.
Current state:
There is currently no alert or limitation in creating an obstetrical file for a patient who already has an active pregnancy.
The only way of correcting this type of mistake is to reopen all notes from the one where the erroneous obstetrical file was created to the most recent one, and delete the form instances one by one from most recent to the note where it was created incorrectly, and then make sure the information is properly added to the original obstetrical file.
