Option to add the letter at the end of the NAM for patient with an immigrant health card.

Task affected: Creating a new patient file or modifying the health card number

Impact: It would allow the display of the health card number for immigrants in the patient banner, and when the forms autopopulate the patients demographics.

Current state: There's a 12 letter/number limit, and users need to add this information somewhere else.
