Change the position of the 'Recipient' field back to the first section of the Consultation Request

Affected user(s): Doctors and users creating clinical notes and consultation requests

Task affected: Adding a consultation request

Impact: Since the position of the Recipent field was moved to the CONSULTATION REQUEST STATUS section, doctors can more easily forget this field. Also, the way the form appeared before looked better, according to client.

Current state: The field to add the recipient is only found lower in the consultation request.
