Change the current "speciality" list format in Consultation requests to a dropdownlist

Currently, the format of the "speciality" list available to our clients in the Consultation Requests section of the notes, is only showing a few specialities and is rather static. The client is asking for the list available to them to be in a "dropdown" format, instead only being able to go down in the current list by typing key words. That way, they can browse up and down in the list as they wish.
