Portal: Add more distinct icons for the Portal (e.i: green checkmark, orange icon, etc)

Request: To add a new logo be created to separate the difference between waiting for the patient to indicate the Portal is pending set up and patient linked to another portal account.

Currently: Both icons will be the orange icon on the dark blue Portal icon.
