Pre-Visit questionnaire to collect patient data

Affected user(s): MYLE users

Task affected: The client asks us to create a questionnaire that would enable us to collect certain information from the patient, such as allergies, history (personal, Chx, family), over-the-counter medications or natural health products.
In the same way as the COVID/Rougeoles questionnaires, it could be sent to patients before their appointments, to optimize appointment times.
Following the return of the questionnaire, they could even be pre-listed (e.g. in green) in the medical summary section of the file, and the clinician could take over this information directly (e.g. click to confirm and include it in the file, so that it appears in blue as if the clinician had added it). Information not approved by the clinician would not be included in the note (if, for example, it is not relevant).

Le client demande de créer un questionnaire qui permettrait de collecter certaines informations du patient, comme les allergies, les antécédents (personnels, Chx, familiaux), les médicaments de vente libre ou produits de santé naturels.
De la même manière que les questionnaires COVID/Rougeoles, il pourrait être envoyé au patient avant son rendez-vous de manière à optimiser la durée de ceux-ci.
Suivant le retour du questionnaire, ils pourraient même être pré listés (p.ex en vert) dans la section sommaire médical du dossier et le clinicien pourrait reprendre directement ces informations (p.ex cliquer pour confirmer et l'inclure au dossier, pour qu'il apparaisse en bleu comme si le clinicien l'avait ajouté). Les informations non approuvées par le clinicien ne feraient pas partie de la note (si, p.ex, ils ne sont pas pertinents).

Current state The client is considering the use of such a form (e.g. Forms), but direct integration into MYLE would be favorable, especially for confidentiality issues.
