CALENDAR: Add a column for the Portal in the PA LIST so users can see if that patient has an active Portal or not and send an invitation.

Affected User(s):
Receptionists

Task Affected:
Quickly confirming whether patients have a Portal account from the PA list.

Impact:
Increases the number of clicks (and time) required to confirm this information.

Current State:
Receptionists must leave the list view and manually search for each patient in CALENDAR to verify account status.

    This was requested by GMF Sillery, as well as having the option to mark a patient as No-Show from the PA list.
