RAMQ enrolment form (DT9000) - Add option to edit the clinic's name (free text) instead of always using the site's name

• Enrolling patients at RAMQ

Impact:

• Collectif-Médica and CMH are saying that legally, they must use a clinic name that's not the one in Admin > Sites.

Current state:

• The name of the clinic cannot be edited, so Collectif-Médica and CMH can't use the form. Instead, they have to use the editable one found in the clinical note under Forms, which isn't nearly as efficient.
