Allow Configuration of Editable Patient Information Fields for the kiosk

Add the ability to restrict which patient information fields can be edited when the “Allow patients to modify their information” permission is enabled.

For example, allow configuration so that only specific fields—such as the patient’s email address—can be modified by the patient, while all other fields remain read-only.
