Portal account :Display an explanatory message regarding the RAMQ number when creating the portal account

Affected users: Patients creating portal accounts
Task affected: The client would like to display the following sentence: "Si vous n’avez pas de RAMQ, sélectionnez « Autre » dans le champ du numéro d’assurance maladie et inscrivez 00000." (in English: "If you do not have a RAMQ health card number, select “Other” in the health insurance number field and enter 00000." ) when the patient doesn't enter a RAMQ number.
Impact: When creating portal account for the patients.
