Results - Add filter showing "On hold by" for list of clinicians

Currently, there is no filter to show a list of selected clinicians that have put a result on Hold.

This is useful and necessary, especially when the prescribers are Drs/ips that are not MYLE users; the button "On hold" for those results ends up only being in the patient file.
