Web Fax - Add an option to include patient identification on the fax cover sheet

Affected users: Primarily prescribing clinicians (Physicians) and clinical staff who send prescriptions or medical documents via the MYLE integrated fax system.

Task affected: This occurs during the fax sending step (sending window), specifically when the user selects the "Fax cover sheet" option. Currently, patient details are not automatically populated, and there is no option to toggle them on.

Impact: There is a moderate clinical risk and a loss of productivity. If pages (such as special instructions) become separated at the recipient's end (e.g., pharmacy), the lack of patient ID on the cover sheet makes it difficult to re-link documents. This leads to medication safety risks or requires the clinic to handle follow-up calls to clarify identity.

Current state: Clinicians must manually type the patient's name on the cover sheet, which is frequently overlooked. The user is requesting an optional checkbox or toggle during the fax sending process to pull patient info onto the sheet at their discretion.
