Affected users: Healthcare professionals (Physicians and Nurse Practitioners) who prescribe complex medication regimens or titration schedules.
Task affected: Prescription module, specifically when using the "Followed by" (titration) feature and when defining frequencies for medications requiring unequal split doses (e.g., BID with different quantities morning and night).
Impact: High/Critical. It currently presents a significant medico-legal risk and a threat to patient safety due to potential under-dosing or over-dosing if a pharmacist misinterprets the consolidated instructions at the bottom of the script. It also causes significant productivity loss for both the clinic and the pharmacy due to frequent back-and-forth clarifications.
Current state: Currently, specific instructions entered for a titration step are moved to a global "Instructions" section at the end of the prescription rather than remaining attached to the specific step. For unequal doses (e.g., 25mg AM / 50mg PM), users must manually calculate the total daily dose and explain the breakdown in text, which is prone to error.
Competition consideration: Previous EMRs the requester had (and other competitors) offer a "concurrent" or "split-dose" dosing option that allows clinicians to specify different tablet counts for each time of day (qAM, qPM, etc.) within a single prescription line.
