Affected users: Administrative staff (MOA), clinic management, and healthcare professionals who use Memos to collaborate on patient files.
Task affected: Editing and saving an existing Memo. Currently, when a user updates a Memo, their name completely overwrites the name of the previous user who created or last modified it.
Impact: High. This behavior destroys accountability and audit trails, as users cannot track who originally entered the information once a Memo has been updated.
Current state: No efficient workaround exists. Users must manually write their names and dates within the text box of the Memo to preserve a history of who wrote what, which is inefficient and prone to omission.
