Forms : the Preferred phone to be used by default in the forms.

Affected users: all users managing the contact with the patients.
Task affected: The client would like the forms to use the Preferred Phone number regardless of its type (cell or phone)
Impact: The forms may contain a number not preferred by the patient.
Current state: The forms use the cell numbers even if it's not the preferred number.
