Forms - add ability for users to upload and create their own
users would like the flexibility to upload and create their own forms. With custom fields; drop-downs, free text, radio buttons, etc...

very similar request, would like to have a way to create forms that are flexible and malleable depending on patients (drag-drop results into the doc, merge different documents/graph in a specific manner to create something presentable as a report for the patients).

Hence they would like to incorporate something of the sort in MYLE.
