Add a field in Administration>Logs to track when a Signature is preserved or revoked in a file

If a signature is saved, and then revoked, it is not tracked in the logs in MYLE.
For example, if a physician saves a signed document, and another user prints or faxes it, and then the physician revokes the signature.
The logs do not capture that the document has a preserved signature on it or that it was removed.
