Add the new form AH-888 as a button in Patient Profile-Enrolment

The RAMQ now requires another mandatory form to be able to enrol a patient, the patient has to authorize the clinic to send his information to the RAMQ.

By going to a Patient Profile - Enrolment and clicking on Generate Form the form 4096 opens, but this form must always be accompanied by another one which is the AH-888.

Clinics would like us to add a button next to Generate Form that opens the form AH-888.

Please note that the Form AH-888 is already available in MYLE as a dynamic form under the name :
