Ability to see the "Referred by" field to physician's calendar printout

Ms Julie Leblond from the clinic "Evo" said that they have to add manually the "Referred by" section on every print, and it's becoming a real issue for them.
They would like to have a field like "notes" so they can add additional information for each appointment.
