Add extra Appointment state

User would like to have an Appointment state that indicate "Note to be completed" as an indication that the patient was seen and all thats left is the note to be completed, this would help them to be more organized when clinicians have a very busy they with notes that are not completed to go back in the calendar and have a quick view of what is left to be concluded as for patient notes.
