To have the fax number instead of the phone number of a recipient on a consultation request

When you print a consultation request, the phone number is showing next to the recipient's name instead of the fax number. It's not helpful for secretaries if the doctor prints out the request and they have to fax it. To find the fax number, they have to go in a test file, create a note and search for the doctor's info.
