Add the requesting staff to the preventive activities

For UMF Moncton and any other clinics with residents, the residents would need to be able to add the supervisor's name (requesting staff) to preventive activities just like they do for prescriptions and exam/consultation requests. The supervising doctor needs to be able to keep track of the preventive activities created by their residents and right now they can't. They can access them in the portfolio, but unlike in Qc, NB residents are supervised by different doctors and are not assigned their own patients.
