Add the ability to have more than one fax contact in the Directory for the same intervenor.

Please add the ability to have more than one fax contact in the Medfar Dictionnary for the same intervenor.

Presently if a doctor works in more than one clinic, we can only enter the fax number for one location.

This is not ideal as it then requires the doctor to receive documents at Location A, when they were meant for Location B
