Adding a comment and updating the status of the preventative activities via the Portfolio

When MOAs are following up with preventative activities that are due via the Portfolio, they would like to be able to update the status and add a comment via the Portfolio rather than having to do it via the patient's chart.

Consultation Request - Move '' recipient '' section back up the first half instead of below the '' status '' section.

Prescriptions: users would like to display the first medication entered on the prescription at the top by default.
