Consultation Request - Move '' recipient '' section back up the first half instead of below the '' status '' section.

Requested by: Giselle Pierre - wbw User wants to have the recipient section in the top half of the pop-up when creating the request instead of below the '' status'' section - similar to how it was before the improvements.
