WCB forms - allow more space within the typed in section of Form 8 and Form 11

Currently: The textboxes limit the amount of text that can be written in these forms. User would appreciate the option to have a '' continued '' section or more space to include further information in those textboxes.

Ability to add more than one preferred pharmacy to a patient profile.

This option would have added value for patients who have two addresses, thus two pharmacies. For example, children whose parents have joint custody or patients who live at their cottage during the summer.
