Add an option to hide "Signed on"

PATIENT: Users would like an option to hide the section "Signed on" when they print or fax notes in MYLE so the receiver won't know how long it took them to complete the note by seeing the date it was signed (approved by the user).

When printing the complete patient file, page # and total pages printed are mandatory to be sure nothing is missing. It could be interresting to add this kind of counter at the bottom : INFORMATION CONFIDENTIELLE - PAGE 1/100 for example
