Add the ability to include forms in the note when selecting print from the note

When you go into a note to print it, there is no mention in the printout for any forms added in the note

To go a patient file > Click on any approved note with a form in it
When it prints, there is no mention of any form in the note.
If it is not possible to include the form, can we at least make it so that the printout version shows the name of the form included in the note.
