Simplifying Portal Account Creation

– Task affected: Patient Portal account creation. The client reports that there are too many fields to fill by the patients. The client wants to keep mandatory only the name and phone number.

– Impact : The clinic is loosing possible patients because of the many fields that should be filled in order to create an account and be able to book an appointment.
